9. A registered medical practitioner shall certify in the prescribed form —
that the consent in writing of a person under section 6 or 7 was given in the practitioner’s presence;
that the practitioner explained to the person before the consent was given the nature and effect of the removal from the body of the person specified in the consent of the tissue so specified; and
that the practitioner is satisfied —
that the person giving the consent was an adult, at the time the consent was given; capacity to consent; and
that, at the time, the person was of sound mind and did not lack
that the consent was freely given.
