Procedures for making disclosures

A disclosure shall be in writing and shall contain, at a minimum, the information specified in Schedule 1.
Notwithstanding subsection (1), if a disclosure is made orally, the designated authority shall, within twenty-four hours after receiving the disclosure, cause the disclosure to be reduced into writing containing the same particulars as are specified in subsection (1).
